Your initial appointment will consist of a consultation to discuss and learn about your current condition, and we will explain your diagnosis and treatment options. Occasionally, surgery can be done the same day as the consultation. However, an extensive medical history or complex treatment plan will require an initial consultation and follow-up appointment(s) to provide treatment.
Please assist us by providing the following information at the time of your initial consultation:
- Please bring this referral slip and any radiographs (if applicable) to your consultation/surgery.
- Please alert the office if you have a medical condition that may be of concern.
- Please bring your medication list.
- Please bring complete dental/medical insurance information of you/spouse/ children/parents.
- Please remove contact lenses prior to surgery
- Please let us know any changes in health upon arrival, i.e. fever, chills, cough, running nose, etc.
- Please give us 24 hours advance notice if cancellation.
All patients under the age of 18 years of age must be accompanied by a parent or guardian.
An initial consultation and physical examination is mandatory for patients undergoing intravenous anesthesia. You must not eat or drink 6 hours prior to your surgery. You will also need an adult to drive you home.
